Getting ready for the appointment?

Bring all documents listed here that applicable to your tax situation.

  • Proof of identification: driver’s license, passport or Nexus card.

  • Hard copy of the Social Security cards or proof of ITIN for you, your spouse and dependents. We cannot help you if you don’t bring the physical card, even if you know your SSN or ITIN.

  • The Intake/Interview form, which is linked here, must be filled out for every return we prepare on site. Please download, print, and try your best to answer all questions on the first three pages. If you don’t have a printer at home, please transcribe your answers to a hard copy of this form on our VITA site. Filled out this form before the appointment will help VITA tax preparers work more efficiently and minimize any potential tax errors.

  • Documents about all your wages and earnings listed on the intake form (Form W-2, W-2G, 1099-R,1099-MISC, 1099-NEC. etc.);

  • Interest and dividend statements from your banks and stock trading platforms (1099-DIV, 1099-INT), banks don’t issue 1099-INT if interest amount is less than 10 dollars;

  • Capital gain or loss report from your stock trading platforms (1099-B). A list of detail stock trading transactions is not a acceptable replacement for the 1099-B form;

  • Proof of the Children Daycare expenses from the provider;

  • Forms 1095-A, the Health Insurance Marketplace Statements;

  • Heath Saving Account contributions and distribution forms/proofs;

  • Proof of all other expenses listed on the intake form;

  • A copy of last tax year’s federal and state returns, if available; and

  • Bank account routing and account numbers for direct deposit/debit, such as a blank check.
